Who is this form for?

This is the student registration form for the Fairbanks Suzuki Institute. On this form you will be able to sign up multiple students for the core curriculum, electives and graduduation. You will also be able to sign up siblings for sibling electives. Any student planning to attend this program must register on this form.

3. GRADUATION

If you would like to participate in Suzuki Graduation, please discuss it with your private teacher and start the preparation process early. You can see the Graduation Requirments here.

- Deadline for graduation materials submission is March 1.
- Graduation fee is $50 per level. If you plan to graduate from multiple levels it will be $50 per level.
- Important: This form is for students that are already registered for our program on the previous page. So be sure you have add your graduate to Program Registration Page.

4. SIBLING ELECTIVES

Siblings of enrolled FSI students may register for Dalcroze and/or Lunch Games on a space-available basis.

- Siblings must be 5+, and good listeners so as not to be disruptive to the group learning environment.
- Fees: $75-$100/elective.

5. ADDITIONAL T-SHIRTS

Each student who enrolls in an FSI program will get a t-shirt with their registration. If you would like to order additional t-shirts for siblings, parents or grandparents, please order now with registration. There will be a very limited supply of extra t-shirts available for purchase at FSI and the only way to guarantee a t-shirt if you are not an enrolled student is to order now.

6. WAIVER & ACKNOWLEDGEMENT

FSI Waiver and Mass media, print, graphic acknowledgement.

- Attendance at FSI means you agree to this.
